All new students must have on file by the first day of school:
A copy of a legal birth certificate or other reliable proof of age and identification
A current Kentucky certificate of immunization
Proof of a preventative health care examination conducted within six months prior to initial entry into school (and another preventative health care examination within one year prior to entry into the 6th grade)
For Catholic students only: a copy of Baptismal certificate if Baptism was not received at Good Shepherd Catholic Church.
In addition:
Proof of an eye examination between ages 3-6 by an optometrist or ophthalmologist documented on the Kentucky School Eye Exam Form must be presented to the school upon entry to kindergarten.
Proof of a dental screening or examination by a dentist, dental hygienist, physician,registered nurse, advanced registered nurse practitioner or physician assistant, documented on the Kentucky Dental Screening Form. This form must be presented to the school no later thanJanuary 1 of the first year that a 5- or 6-year-old child is enrolled. (Kindergarten)
Students entering sixth grade must have proof of a preventative health care examination conducted within one year prior to entry into the 6th grade. This must be on file by the first day of the sixth grade year.
ALL students must have a current immunization certificate on file. If your child's certificate has expired, please submit an updated copy to the school office.
